Resolution - Public Health Services: Consider a resolution authorizing the Mayor to execute, for and on behalf of the City of Lubbock, Amendment No. 3 to the Public Health Emergency Preparedness Cooperative Agreement, Contract No. 537-18-0127-00001 and any associated documents, with the State of Texas acting by and through its Department of State Health Services (DSHS), for supplies related to the implementation of the agreement.

Item Summary
This amendment adds $5,582 to the contract.  The total amount of the contract is now $769,215, with $261,793 added for FY 2019-20.  The contract period has been extended to June 30, 2020. 
 
This contract with the Department of State Health Services requires that the City of Lubbock shall administer programs and perform activities in support of the Public Health Emergency Preparedness Cooperative Agreement from the Centers for Disease Control and Prevention (CDC).  The Statement of Work is attached.
Fiscal Impact
Expenditures to this program will not exceed the $769,215 provided in the State contract.  A 10% match is required, which will be met with Information Technology charges paid with general funds.
