Resolution - Public Health Services:
Consider a resolution authorizing the Mayor to execute Service Agreement 14465 with Triple Aim Ventures, LLC, d/b/a/ TAV Health, to provide consulting services for the City of Lubbock Health Department.
A Request for Proposal (RFP) was published in order to select a consultant to assist the Health Department in addressing social determinates of health, and developing a community navigation system. Two firms responded to the RFP, and both were reviewed by the selection committee. The selection committee, appointed by the Director of Public Health, reviewed and rated the qualifications of the proposals. The contract is for 1 year, with the option of a 6-month extension.
The evaluation criteria in the RFP included:
1) Qualifications and Experience - 30%
2) Rates and Expenses - 30%
3) Methodology including technical approach and understanding of the scope of the project - 25%
4) References - 15%
The selection committee evaluated the proposals and arrived at the following scores:

Consilience Group, LLC
The committee recommends TAV Health for a 1-year contract with compensation of $49,000, plus itemized and approved expenses up to $6,900.
This contract is not to exceed $55,900, and the contract is funded by a grant from the Kresge Foundation.
